Unsafe conditions are physical hazards in the workplace that increase the risk of injuries and accidents.
Recognizing and reporting these hazards immediately you spot them is a shared responsibility, not just the safety team’s job. 
And it's always advisable to inspect your work area and report or correct the risks before starting your work. 

Common Unsafe Conditions to Watch For:
Poor Housekeeping: Wet floors, cluttered walkways, and debris.

Defective Equipment: Broken tools, malfunctioning machinery, and inadequate guarding.

Environmental Hazards: Poor lighting, lack of ventilation, or extreme temperatures.

Electrical Hazards: Exposed wires or overloaded circuits. Signs: flickering lights, buzzing noises, burning smells, or appliances losing power. 

Safety Violations: Unsafe work procedures, missing warning signs, or improper storage. E.g. Uneven or Unstable Stacking, Top-Heavy Storage

How to Handle Unsafe Conditions:
Identify and Stop: Pause your work if you spot a hazard.

Report: Immediately notify a supervisor or safety officer.

Correct or Barricade: If safe to do so, fix the issue immediately (e.g., cleaning a spill). 
Otherwise, restrict access to the area with barricades or warning signs.

Investigate: Identify the root cause to prevent reocurrence. 
5 Whys Technique: Ask "why" repeatedly (typically five times) to drill down from the symptom (e.g., oil on floor) to the root cause (e.g., purchasing bought cheaper, leaking gaskets).

Actionable Tip: Before every shift, take 60 seconds to scan your immediate work area for new or unaddressed hazards. If you see something, say something.
